Clinical Design

how do medical teams support change when patients have had long-term embedded behaviors? we know that physical health is mental health so to find improvement we must look at the context of a person’s environment. This includes the medical team environment and the patient’s environment. incorporating integrative behavioral health is a new model of care and requires reframing culture, roles, and delivery models. it also requires interventions to understand and treat depression, anxiety, trauma, and behavior change. what does it really mean to work together as team-based collaborative care? focusing on the common goal, shared treatment plan and unified actions all lead to cohesiveness for all stakeholders.
